General Summary
Under the general supervision of the Administrator, the Social Worker has the primary responsibility of developing and implementing the social services component. In addition to clinical services, this include policy development and the integration of social services with health services.
Essential Functions
Conduct initial psychosocial assessments for all new participants and complete required documentation;
1. Preparing the social work portion of the client plan of care on the basis of the
assessment and providing services, including;
Providing or arranging for individual, family and group counseling in reference
to psychological, social, financial, legal, vocational, and educational needs of
the clients;
2. Assisting with obtaining concrete services; for example, housing, shopping,
clothing, etc.;
3. Referring to and/or developing support groups and educational programs for caregivers and recipients;
4. Arranging and/or providing counseling, crisis intervention, and advocacy for participants and their families;
5. Providing family outreach;
6. Coordinating client's treatment plans with other community resources;
7. Providing in service training to staff on client/ caregiver psychosocial needs;
8. Participating in all client case conferences; for example, pre-admissions and
post admissions, problem-oriented cases;
9. Participating as part of the multidisciplinary team in developing, implementing,
reviewing the patient plan of care;
10.Reassessing the client ongoing basis and revising the social work portion of
the client plan of care;
11.Documenting assessments, evaluations, treatment plans, clinical notes etc. in
the client’s medical records;
12.Developing and implementing a quality assurance program;
13.Participating on professional organizations and seminars;
14.Uphold participant rights, confidentiality, and cultural sensitivity;
15.Coordinating discharge planning for the members including linking them to
necessary community services.
