Win is seeking a compassionate and clinically skilled Client Care Coordinator (LMSW/LMHC) to provide trauma-informed counseling and support services to individuals and families living in our Supportive Housing program. In this role, you will assess client needs, provide short-term therapeutic interventions, connect families to community resources, and help ensure adults and children receive the services they need to achieve stability and well-being.
WHAT YOU WILL DO
Conduct psychosocial assessments to identify the clinical, emotional, and service needs of individuals and families.
Provide short-term counseling, crisis intervention, support groups, and workshops for residents.
Connect clients to community-based mental health, substance use, domestic violence, and other long-term support services.
Collaborate with internal teams and external providers to coordinate care and ensure continuity of services.
Identify and support high-risk clients, including survivors of domestic violence, individuals with mental health or substance use challenges, and families involved with ACS.
Partner with parents, schools, healthcare providers, Early Intervention programs, and community agencies to address the needs of children and families.
Facilitate therapeutic play groups, workshops, and other trauma-informed programming for adults and children.
Participate in case conferences, team meetings, on-call rotations, trainings, and other activities that support client success and program goals.
WHO YOU WILL BE
LMSW or LMHC required within 90 days of employment and maintained throughout employment.
Experience providing clinical services, counseling, and assessments to individuals and families.
Strong knowledge of trauma-informed care, crisis intervention, and community-based service systems.
Experience working with individuals experiencing mental illness, substance use disorders, homelessness, or other complex challenges.
Skilled in facilitating groups, workshops, and psychoeducational programming.
Experience using standardized assessment tools and developing individualized service plans.
Excellent communication, documentation, and relationship-building skills.
