    $70,000-$75,000

     
    THE ROLE
    Win is seeking a compassionate and clinically skilled Client Care Coordinator (LMSW/LMHC) to provide trauma-informed counseling and support services to individuals and families living in our Supportive Housing program. In this role, you will assess client needs, provide short-term therapeutic interventions, connect families to community resources, and help ensure adults and children receive the services they need to achieve stability and well-being.
     
    WHAT YOU WILL DO
    • Conduct psychosocial assessments to identify the clinical, emotional, and service needs of individuals and families.
    • Provide short-term counseling, crisis intervention, support groups, and workshops for residents.
    • Connect clients to community-based mental health, substance use, domestic violence, and other long-term support services.
    • Collaborate with internal teams and external providers to coordinate care and ensure continuity of services.
    • Identify and support high-risk clients, including survivors of domestic violence, individuals with mental health or substance use challenges, and families involved with ACS.
    • Partner with parents, schools, healthcare providers, Early Intervention programs, and community agencies to address the needs of children and families.
    • Facilitate therapeutic play groups, workshops, and other trauma-informed programming for adults and children.
    • Participate in case conferences, team meetings, on-call rotations, trainings, and other activities that support client success and program goals.
     
    WHO YOU WILL BE
    • LMSW or LMHC required within 90 days of employment and maintained throughout employment.
    • Experience providing clinical services, counseling, and assessments to individuals and families.
    • Strong knowledge of trauma-informed care, crisis intervention, and community-based service systems.
    • Experience working with individuals experiencing mental illness, substance use disorders, homelessness, or other complex challenges.
    • Skilled in facilitating groups, workshops, and psychoeducational programming.
    • Experience using standardized assessment tools and developing individualized service plans.
    • Excellent communication, documentation, and relationship-building skills.
