    Client Care Coordinator

    Change Lives, Providing Clinical Support

    $65,000-$70,000

    *LMSW or LMHC Licensing required within 90 Days*

     
    THE ROLE
    Win is seeking a compassionate and skilled Client Care Coordinator (LMSW or LMHC) to provide clinical services to families and individuals residing in our Transitional Housing programs. In this role, you will help clients address barriers to stability through assessment, counseling, crisis intervention, and connection to long-term community-based services. Working closely with shelter staff, external providers, and families, you will play a vital role in supporting recovery, resilience, and housing stability.
     
    WHAT YOU WILL DO
    • Conduct psychosocial assessments to identify client needs and develop appropriate service recommendations
    • Provide short-term counseling, crisis intervention, and supportive clinical services to individuals and families
    • Facilitate therapeutic groups, workshops, and educational programming that promote wellness and stability
    • Connect clients to community-based mental health, medical, educational, and supportive services for long-term care
    • Collaborate with internal teams and external partners to ensure coordinated, client-centered service delivery
    • Identify and respond to high-risk situations involving domestic violence, mental health concerns, substance use, child welfare involvement, and other critical needs
    • Support children and families through observations, service planning, referrals, and trauma-informed interventions
    • Advocate for clients while helping them navigate systems and access resources that support long-term success
     
    WHO YOU WILL BE
    • LMSW/LMHC must be obtained within first 90 days of employment and maintained throughout Win employment.
    • Demonstrated experience facilitating groups and developing group curricula.
    • Experience with standardized assessment tools.
    • Experience working with individuals with mental illness and substance abuse, preferably with homeless adults and at-risk populations.
    • Performs job duties that include work requiring advanced knowledge in the field of social work/mental health.
    • Must be available for on call rotation.
    • Must attend required internal and external meetings as well as trainings assigned by the supervisor.
    • Use advanced knowledge acquired by a prolonged course of specialized intellectual instruction.
