A Brief Overview

The Oncology Social Worker provides comprehensive psychosocial support and case management services to patients and their families throughout the cancer care continuum. This role combines clinical counseling with care coordination to address the emotional, social, financial, and practical challenges associated with a cancer diagnosis. The Oncology Social Worker conducts psychosocial assessments, provides individual, family and group counseling, develops individualized care plans, connects patients with community and financial resources, and collaborates with the interdisciplinary health care team to promote optimal patient outcomes, quality of life, and continuity of care.  



What you will do

 

• Conduct comprehensive psychosocial assessments to identify patients' emotional, psychological, social, financial, and practical needs 
• Provide individual, family, and group counseling to assist with adjustment to cancer diagnosis, treatment, survivorship, recurrence, and end-of-life care
• Assess patients for suicide risk, domestic violence, substance use, and other psychosocial concerns, implementing appropriate interventions and referrals
• Facilitate advance care planning discussions and provide support regarding goals of care and healthcare decision-making
• Develop individualized care plans that address identified psychosocial barriers to treatment and recovery
• Coordinate services across the continuum of care, including outpatient and community-based resources
• Connect patients and families with financial assistance programs, transportation, lodging, insurance resources, disability benefits, and other supportive services
• Serve as an active member of the multidisciplinary oncology team, participating in case conferences, tumor boards, care planning meetings, and interdisciplinary conferences as needed
• Collaborate with physicians, advanced practice providers, nurses, rehabilitation specialists, palliative care providers, financial counselors, and community agencies to coordinate comprehensive patient care
• Maintain timely, accurate, and comprehensive documentation in the electronic medical record
• Participate in quality improvement initiatives designed to enhance psychosocial oncology services and patient outcomes
• Participate in continuing education and maintain required licensure and certifications
• Participate in departmental meetings, organizational committees, and community outreach activities as assigned
• Assist with program development and evaluation of psychosocial oncology services
• Perform other related duties as assigned to support the mission and goals of the organization

• Skills: Analytical and problem solving along with excellent verbal and written communication skills. Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.).

If you are hired at Holy Name, your final base compensation will be determined based on factors such as employment status (Full/Part-Time or Per Diem) skills, education, and/or experience. In addition to those factors – we believe in the importance of pay equity and consider any internal equity of our current team members as a part of any final offer. Pay Range: $72,800 - $86,320
