A Brief Overview
The Oncology Social Worker provides comprehensive psychosocial support and case management services to patients and their families throughout the cancer care continuum. This role combines clinical counseling with care coordination to address the emotional, social, financial, and practical challenges associated with a cancer diagnosis. The Oncology Social Worker conducts psychosocial assessments, provides individual, family and group counseling, develops individualized care plans, connects patients with community and financial resources, and collaborates with the interdisciplinary health care team to promote optimal patient outcomes, quality of life, and continuity of care.

Minimum of two (2) years of experience in a health care or community-based setting providing psychosocial support, counseling, and/or case management services to individuals with chronic or complex medical conditionsrequired |
