Position Summary:

In this role, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your efforts will help patients overcome barriers, access community resources, and ensure continuity of care across inpatient and outpatient settings.
What You'll Do:
Conduct comprehensive psychosocial assessments for patients and families.
Develop treatment plans that address emotional, social, and resource-related needs.
Provide brief, solution-focused counseling and facilitate discharge or care planning.
Advocate for patients and coordinate referrals to community-based services.
Collaborate effectively with interdisciplinary care teams.
Document all assessments, plans, interventions, and outcomes clearly and promptly.
Meet departmental productivity expectations and performance standards.
What You'll Bring:
Education: Master's Degree in Social Work or a related field from an accredited institution.
Licensure: Must be eligible for licensure as a Limited Licensed Master Social Worker (LLMSW) or Licensed Master Social Worker (LMSW) by the Michigan Board of Social Work.
Experience: Minimum of 2 years of social work experience in an acute inpatient setting.
Key Competencies:
Strong psychosocial assessment skills and clinical judgment.
Ability to navigate complex family dynamics and high-acuity situations.
Excellent crisis support and problem-solving abilities.
Outstanding interpersonal and empathetic communication skills.
Proficiency in engaging patients and care teams in effective treatment planning.
Knowledge of community resources and continuity of care principles.
Accurate, timely documentation in medical records and reporting systems.
