Job Title:LMSW - Social Services
Location: Anchorage, AK 99508
Schedule: Day Shift | 4x10-Hour | 07:00 AM - 05:30 PM | Every Other Weekend
Pay Range: $2,000 - $2,300 Weekly
Assignment Length: 26 Weeks

Key Responsibilities:

• Provide psychosocial and emotional support to patients and families.
• Complete comprehensive psychosocial and biopsychosocial assessments.
• Develop and coordinate individualized discharge plans in collaboration with patients, families, RN Case Managers, and the care team.
• Provide brief clinical interventions, crisis intervention, and safety planning when appropriate.
• Assist patients and families with resource navigation and coordination of community-based services.
• Coordinate care with internal and external healthcare teams and community resources.
• Document assessments, interventions, care plans, and patient responses accurately.
• Identify and report safety concerns or suspected abuse in accordance with applicable regulations and state requirements.

Required Skills & Experience:

• Minimum 2 years of social work experience preferred.
• Acute care hospital experience is REQUIRED.
• Experience with inpatient medical social work, discharge planning, care coordination, and psychosocial assessments.

Education & Licensure:

• Master's degree in Social Work (MSW) from a CSWE-accredited school of social work.
• Alaska LMSW license required; candidates without the license in hand may apply for a temporary LMSW license.
