Pride Health is actively hiring a Registered Nurse (RN) to join our client's In Home Health Team.

 

Location: Westborough, MA 01581

Patient Care Setting: Home Health
Weekly Hour Guarantee: 32 Hours (40 Hours Available Based on Caseload)
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Productivity Expectations:

  • 26 Points in 32 Hours
  • 33 Points in 40 Hours

Coverage Area: Leominster and surrounding towns
On-Call Requirement: None
Weekend Requirement: None (flexibility available for occasional schedule adjustments with valid notice)
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Casual Professional Attire or Scrubs

Position Overview

We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the Leominster service area. The RN will be responsible for comprehensive patient assessments, care planning, case management, OASIS documentation, and coordination of care to promote positive patient outcomes and reduce hospitalizations

Key Responsibilities

  • Perform comprehensive nursing assessments, admissions, Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges, and routine skilled nursing visits.
  • Develop, implement, and update individualized patient care plans in collaboration with physicians, caregivers, and interdisciplinary team members.
  • Monitor patient progress, identify changes in condition, and communicate findings to physicians and clinical leadership as appropriate.
  • Provide skilled nursing interventions including medication management, wound care, disease management education, and patient/caregiver training.
  • Complete accurate and timely OASIS assessments and clinical documentation in compliance with agency, state, and federal regulations.
  • Coordinate care with therapists, social workers, physicians, community resources, and family members to ensure continuity of care.
  • Educate patients and caregivers regarding diagnoses, medications, treatment plans, safety measures, and self-management strategies.
  • Maintain productivity standards while delivering high-quality, patient-centered home health services.

Required Qualifications

  • Active Massachusetts Registered Nurse (RN) License.
  • Previous Home Health Nursing Experience.
  • Strong OASIS Assessment and Documentation Experience.
  • Excellent clinical assessment and care coordination skills.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to work independently and manage a field-based caseload.

Preferred Qualifications

  • Experience with admissions, recertifications, and discharge assessments.
  • Strong interpersonal and relationship-building skills.
  • Excellent communication and patient education abilities.
  • Effective time-management and organizational skills.
  • Ability to collaborate successfully within a multidisciplinary care team.
  • Knowledge of regulatory and home health compliance requirements.
