Home Health Registered Nurse (RN)

Location: Everett, WA 98204

Schedule: Full-Time | Flexible schedule with local travel

Job Summary

We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will assess patient needs, develop and implement individualized Plans of Care (POC), coordinate services with the interdisciplinary team, and ensure high-quality, compassionate patient care.

The ideal candidate will have strong Home Health, OASIS, and Homecare Homebase (HCHB) experience and the ability to work independently while collaborating effectively with patients, families, physicians, and other healthcare professionals.

Key Responsibilities

• Perform comprehensive assessments of home health and/or hospice patients and identify physical, psychosocial, and environmental needs.
• Develop, implement, document, and evaluate individualized Plans of Care (POC) to ensure quality and continuity of care.
• Assess physical, psychological, social, spiritual, and cultural factors and incorporate findings into patient care plans.
• Provide skilled nursing care while maintaining accurate and timely clinical documentation and patient records.
• Follow infection-control practices and OSHA guidelines to protect patients and staff.
• Educate patients and families regarding medications, treatment plans, diet, home safety, disease management, and self-care.
• Supervise and provide clinical direction to HHA/STNA/LVN staff as appropriate.
• Assess and reassess pain and implement appropriate pain-management interventions.
• Coordinate referrals and communicate effectively with patients, families, physicians, administration, and interdisciplinary team members.
• Monitor assigned cases for compliance with applicable third-party payer requirements.
• Participate in interdisciplinary team meetings and case conferences as required.
• Perform on-call responsibilities and provide patient/family support as assigned.
• Perform other related duties as assigned.

Required Skills & Qualifications

• Current Registered Nurse (RN) license in the state of Washington.
• Homecare Homebase (HCHB) experience – Required
• OASIS experience – Required
• 2+ years of Home Health experience – Required
• 1–2 years of RN clinical care experience – Required
• Current CPR certification – Required
• Current driver's license – Required.
• Reliable transportation and valid auto insurance – Required.
• Strong knowledge of medications and safe medication administration.
• Experience with venipuncture, IV care, wound care, phlebotomy, and routine medical/surgical nursing skills.
• Knowledge of catheter and drain care and safe use of durable medical equipment in the home setting.
• Strong assessment, communication, organizational, and documentation skills.
• Ability to work independently and as part of an interdisciplinary team.
• Ability to maintain patient confidentiality.
• Ability to travel locally and work a flexible schedule.

Education

• Required: Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma program.
• Preferred: Bachelor of Science in Nursing (BSN).
