Registered Nurse (RN) – Long-Term Care / Rehabilitation
We are seeking a compassionate and dedicated Registered Nurse (RN) to provide quality nursing care in a Long-Term Care and Rehabilitation setting.
Job Responsibilities
Provide direct nursing care to residents in an LTC/Rehab environment.
Assess residents, monitor vital signs, and recognize changes in condition.
Administer medications and treatments as ordered.
Develop and implement individualized resident care plans.
Monitor residents’ progress and support rehabilitation and restorative care goals.
Collaborate with physicians, therapists, CNAs, and other interdisciplinary team members.
Provide wound care, pain management, fall prevention, and infection control as needed.
Assist residents recovering from illness, injury, surgery, or hospitalization.
Document assessments, care provided, medications, and resident progress accurately in the EMR.
Supervise and guide CNAs and other nursing staff.
Communicate effectively with residents, families, providers, and the healthcare team.
Promote resident safety, dignity, independence, and quality of life.
Qualifications
Current and unrestricted RN license in the applicable state.
Previous LTC, skilled nursing, rehabilitation, or post-acute care experience preferred.
Strong assessment, communication, documentation, and organizational skills.
Ability to work effectively in a fast-paced, team-oriented environment.
