Compensation: $135,000 – $150,000 annually (based on experience, plus potential bonuses/incentives)

Job Summary

We are seeking a compassionate and dedicated Primary Care Nurse Practitioner to join a growing outpatient healthcare team in the Bronx, NY. This role offers the opportunity to provide comprehensive primary care services to a diverse patient population while collaborating with experienced physicians and healthcare professionals. Key Responsibilities

• Perform comprehensive patient assessments, physical examinations, and evaluations.
• Diagnose, treat, and manage acute and chronic medical conditions.
• Develop individualized treatment plans and monitor patient progress.
• Obtain and document complete medical, family, and social histories.
• Order, interpret, and evaluate laboratory tests, imaging studies, and other diagnostic procedures.
• Prescribe medications and other appropriate treatments in accordance with state regulations.
• Administer vaccinations and preventive healthcare services.
• Educate patients on disease prevention, nutrition, lifestyle modifications, and medication management.
• Coordinate care with physicians, specialists, and other healthcare providers.
• Maintain accurate and timely documentation within the electronic medical record (EMR/EHR).
• Ensure high-quality, patient-centered care while adhering to clinical guidelines and best practices.

Qualifications

• Master's Degree from an accredited Nurse Practitioner program (Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner).
• Active New York Nurse Practitioner license.
• National Board Certification (AANP, ANCC, or equivalent).
• Strong clinical assessment and diagnostic skills.
• EMR/EHR experience preferred.
• Bilingual (Spanish) is a plus.
