PRIDE Health is seeking a local contract nurse RN Home Health for a local contract nursing job in Cullman, Alabama.

Position Overview

Seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality home health services to patients throughout Cullman, Blount, Winston, and Walker Counties. This position is responsible for delivering skilled nursing care, conducting patient assessments, managing care plans, and collaborating with physicians and interdisciplinary team members to ensure positive patient outcomes. The ideal candidate will have strong wound care and IV therapy experience and thrive in an independent, field-based environment.

 

Schedule & Work Details

  • Coverage Area: Cullman County, Blount County, Winston County, and Walker County, AL
  • Schedule: Monday through Friday, 8:00 AM to 4:30 PM
  • Weekly Hours: 40 hours
  • Pay Rate: $51.00 - $53.00 per hour
  • Productivity Expectation: 30 points per week
  • On-Call Requirement: As requested by the agency
  • Mileage Reimbursement: Yes, reimbursed between patient visits

Key Responsibilities

  • Provide skilled nursing care to patients in their homes in accordance with physician orders and agency policies.
  • Conduct comprehensive patient assessments and ongoing evaluations of health status and care needs.
  • Develop, implement, and update individualized plans of care.
  • Perform wound care treatments and management according to established clinical protocols.
  • Administer and monitor IV therapies, including patient education and complication management.
  • Monitor patient progress and communicate significant changes to physicians and appropriate care team members.
  • Educate patients and caregivers on medications, treatments, disease management, and health maintenance.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Coordinate care with physicians, therapists, aides, and other interdisciplinary healthcare professionals.
  • Support quality improvement initiatives and ensure compliance with agency, state, and federal regulations.
  • Manage daily travel efficiently throughout the assigned coverage territory.

Required Qualifications

  • Active Alabama Registered Nurse (RN) license in good standing.
  • Experience with wound care and IV therapy.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and patient education abilities.
  • Ability to work independently and effectively manage a home health caseload.

Preferred Qualifications

  • Previous home health nursing experience.
  • Experience utilizing Homecare Homebase (HCHB).
  • Knowledge of home health regulations, documentation standards, and best practices.
  • Strong care coordination and case management skills.
  • Ability to build positive relationships with patients, families, and interdisciplinary team members.
  • Dress Code
  • Navy Blue Scrubs required.
