Position Overview:

We are seeking a dedicated Registered Nurse to provide skilled home health care across a variety of patient needs. This role focuses on delivering high-quality clinical services, including admissions and ongoing patient care management, while ensuring accurate documentation and collaboration with the care team. The ideal candidate is adaptable and comfortable working a flexible schedule that may include days, evenings, weekends, and on-call rotations.

Schedule & Work Details:

• Schedule: Flexible shifts including days, evenings, and weekends
• Weekly Guarantee: 36 hours
• Productivity Expectation: 30 points
• On-Call/Weekend Requirement: Every 3 to 4 weeks when fully staffed
• Mileage Reimbursement: Not provided
• Pay Rate: $51 to $53 per hour

Key Responsibilities:

• Perform Start of Care (SOC), Resumption of Care (ROC), and recertification visits
• Conduct comprehensive patient assessments and develop individualized plans of care
• Monitor patient conditions and update care plans as needed
• Provide skilled nursing interventions in accordance with physician orders
• Maintain accurate, timely, and compliant documentation
• Collaborate with physicians and interdisciplinary team members to ensure continuity of care
• Educate patients and caregivers on treatment plans, medications, and disease management
• Manage a dynamic schedule and caseload efficiently while meeting productivity expectations

Required Qualifications:

• Active Registered Nurse (RN) license
• Strong clinical assessment and documentation skills
• Ability to work independently and manage a flexible schedule
• Experience with SOC, ROC, and recertifications
• Preferred Qualifications:
• Prior home health experience
• Strong time management and organizational skills
• Excellent communication and interpersonal skills
• Ability to adapt to changing patient and scheduling needs
