Position Overview:
Seeking a skilled and experienced Registered Nurse to provide high-quality home health care to patients in Howard and Carroll County. This field-based role focuses on delivering individualized care, managing complex clinical needs, and supporting patients in a home setting to improve health outcomes and quality of life.
Schedule & Work Details:
Location: Howard and Carroll County coverage area
Schedule: Monday through Friday, 8:00 AM to 4:30 PM
Weekly Guarantee: 32 hours
On-Call/Weekend Requirement: On-call required
Mileage Reimbursement: Yes, between patient visits (first to last patient)
Pay Rate: $51 to $53 per hour
Dress Code: Navy blue scrubs
Key Responsibilities:
Provide skilled nursing care to patients in their homes in accordance with physician orders and plan of care
Perform patient assessments, identify changes in condition, and adjust care plans as needed
Administer medications, treatments, and specialized procedures
Manage complex clinical tasks including central line care, wound care, wound vacs, and catheter care (foley and suprapubic)
Perform venipuncture and specimen collection
Document patient care accurately and timely in accordance with agency and regulatory standards
Collaborate with physicians and interdisciplinary care team to coordinate patient care
Educate patients and caregivers on disease management, medications, and home care techniques
Required Qualifications:
Active Registered Nurse (RN) license in the appropriate state
Prior home health experience required
Strong clinical assessment and documentation skills
Ability to work independently and manage a field-based schedule
Preferred Qualifications:
Experience with pleurex drains
Experience with central line care
Proficiency in wound care and wound vac management
Experience with catheter insertion (foley and suprapubic)
Strong venipuncture skills
Excellent time management and communication skills
Experience with Homecare Homebase (HCHB) is a plus
