Location: Mansfield, OH 44907
Pay Rate: Up to $58.00/hour
Patient Care Setting: Home Health
Coverage Area: Richland County and surrounding counties
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Productivity Expectation: 30 Points
On-Call Requirement: 1 week of on-call coverage per month
Weekend Requirement: 1 weekend on-call rotation per month
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Scrubs
We are seeking an experienced and compassionate Registered Nurse (RN) Home Health Case Manager to provide skilled nursing care and comprehensive case management services to patients in their homes throughout Richland County and surrounding areas. The ideal candidate will have prior home health experience, strong clinical assessment skills, and the ability to independently manage a patient caseload while delivering exceptional patient-centered care.
