Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

Location: Murfreesboro, TN 37129
Pay: Up to $58.00/hour W-2
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Coverage Area: Robertson, Sumner & Cheatham Counties
Productivity: 30 points/week.
On-Call/Weekend: Minimum 1 weekend per month, with LPN backup
Mileage: Reimbursed between patient visits
Scrubs: Solid-colored scrubs

Position Summary

• We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes.
• The RN will assess patient needs, implement individualized care plans, coordinate services, educate patients and families, and evaluate care outcomes.

Key Responsibilities:

• Provide skilled nursing care to patients in accordance with the established plan of care.
• Assess patients' physical, psychosocial, safety, and comfort needs.
• Perform nursing procedures within the RN's scope of training, experience, and licensure.
• Assist with patient rehabilitation and promote optimal health outcomes.
• Observe, document, and report changes in patient condition to the appropriate supervisor and physician.
• Educate patients, family members, and nursing support staff on appropriate health maintenance and care.
• Participate in care planning and evaluate patient outcomes.
• Follow the nursing process and ensure the plan of care is appropriately implemented.
• Maintain accurate and timely clinical documentation.
• Supervise and motivate staff as needed and participate in assessing patient care needs.

Required Qualifications

• Minimum 3 years of RN experience.
• Minimum 1 year of Home Health RN experience.
• Current, active Tennessee RN license.
• Current BLS certification.
• Strong clinical assessment, documentation, and communication skills.
• Ability to travel throughout the assigned service area.
