Job Summary
We are seeking experienced Registered Nurses for a Nurse Assessor position serving patients in the Bronx/Parkchester area. This role involves conducting comprehensive in-home assessments, reviewing patient health needs, and supporting individualized long-term care plans.
Responsibilities
- Conduct comprehensive in-home assessments of patients' health status, strengths, needs, and preferences.
- Review medical records, referrals, and supporting documentation.
- Complete and electronically submit patient assessments.
- Identify care needs and support individualized long-term care service plans.
- Discuss healthcare options, available services, and community resources with patients.
- Coordinate with interdisciplinary care teams and healthcare providers.
- Document assessment findings, concerns, and case notes accurately.
- Support continuity of care and help reduce gaps or duplication in services.
- Conduct both in-field and telehealth assessments.
Requirements
- Current New York State RN license required.
- Minimum 2 years of experience working in a home setting within the last 15 years.
- Recent hands-on experience conducting comprehensive patient assessments required.
- Home Health, CHHA, LHCSA, MLTC, hospice, assisted living, or other home-based care experience accepted.
- UAS-NY assessment experience strongly preferred.
- Bilingual Spanish-English proficiency strongly preferred.
- BLS required before the start date.
- Must be comfortable working independently in a fast-paced, technology-driven environment.
- Must be available for hybrid telehealth and in-field assessments.
- Candidates cannot perform UAS assessment assignments elsewhere and must complete a conflict-of-interest form before starting.
