Registered Nurse - Home Health/Hospice
ASSIGNMENT LENGTH: 3 months.
LOCATION: San Diego, CA 92111 – REQUIRED travel for home site visits
SCHEDULE: 8:00am-5pm Mon-Fri.
EXPERIENCE REQUIRED: At least 1 year as an RN, in either home or community health, and within the frail or elderly population size.
CERTIFICATIONS REQUIRED:
- Current license to practice as a Registered Nurse in California.
- Current BLS from AHA or Red Cross
Essential Functions of the Job:
- When needed, assesses, using the nursing process, the home care needs of a frail elderly population, and identifies and develops initial plan of care.
- Based on nursing assessment, completes, and submits LOC determination form to Admissions RN supervisor for review.
- Reviews medical records as needed to complete LOC. If medical records can’t be accessed, complete LOC with other relevant information (i.e., hospital discharge information, medications, etc.)
- Conducts initial Home/RN assessments at participant’s home during the team IHA.
- Maintains an up-to-date active list of all LOCs submitted to include date submitted, decision, follow up, etc.
- Follows up on all deferred Prospective Enrollees as needed and resubmits required information to DHCS to request approval.
- Participates in weekly enrollment team meetings as well as IDT and meetings as needed.
- Assists the transitioning of care and monitoring hospitalizations and SNF patients to ensure appropriate transition.
- Communicates and interacts with patient caregivers and/or family members as needed.
- Follows established departmental and position specific policies and procedures and updates as needed.
- Keep abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government/regulatory agencies.
- Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
- Attends meetings as required and participates on committees as directed.
- Performs other duties as assigned or requested.
