ob Summary:
The LTC RN provides nursing care to residents in a long-term care or skilled nursing facility. The RN is responsible for assessing residents, administering medications and treatments, monitoring changes in condition, coordinating care, and supervising nursing staff.

Key Responsibilities:

• Assess residents and monitor their health and changes in condition.
• Administer medications, treatments, and other nursing interventions.
• Develop, update, and follow individualized care plans.
• Monitor vital signs, symptoms, wounds, pain, and overall resident condition.
• Communicate changes in condition to physicians/providers and families.
• Document assessments, medications, treatments, and resident care accurately.
• Coordinate care with LPNs/LVNs, CNAs, physicians, therapists, and other healthcare staff.
• Supervise and delegate appropriate duties to LPNs/LVNs and CNAs.
• Assist with admissions, transfers, and discharges.
• Provide patient and family education and emotional support.
• Respond to emergencies and provide appropriate nursing care.
• Maintain resident safety, dignity, comfort, and quality of life. 

Requirements:

• Active RN license in the applicable state.
• BLS certification preferred/required based on facility.
• LTC, skilled nursing, geriatrics, or similar nursing experience preferred.
• Strong assessment, documentation, communication, and clinical skills.
