Job Overview
We are seeking a Licensed Practical Nurse (LPN) for a night shift contract assignment in a skilled nursing and rehabilitation setting. The LPN will provide direct patient care, medication administration, and ongoing clinical monitoring under RN supervision.
Key Responsibilities
Provide direct patient care in accordance with nursing standards
Administer medications and treatments as prescribed
Monitor and document patient conditions and vital signs
Assist with ADLs including bathing, mobility, and hygiene support
Report changes in patient condition to RN/physician
Maintain accurate and timely clinical documentation
Follow infection control and safety protocols
Collaborate with interdisciplinary care teams

Start Date: 06/05/2026
End Date: 07/24/2026
Shift: Night Shift
Shift Time: 06:00 PM 06:30 AM
Schedule: Sunday, Friday, Saturday
Contract Length: Approximately 7 8 Weeks
Requirements
Active LPN/LVN license required
Prior skilled nursing experience preferred
Strong clinical and documentation skills
Ability to work independently on night shift
Must comply with onboarding requirements
Must bring state or federally issued identification and wear photo badge at all times
COVID-19 vaccination or approved exemption may be required per facility policy
