JOB RESPONSIBILITIES

• Provide direct patient care under RN supervision
• Administer medications and treatments within LPN scope of practice
• Monitor and document patient condition throughout shift
• Perform wound care and basic clinical procedures as assigned
• Support patients with activities of daily living (ADLs)
• Conduct vital signs monitoring and report abnormalities
• Maintain accurate and timely clinical documentation
• Ensure patient safety and infection control compliance
• Collaborate with healthcare team to support care plans
• Respond to patient needs during overnight shift coverage

JOB REQUIREMENTS

• Active Licensed Practical Nurse (LPN) license required
• Graduate of an accredited practical nursing program
• Ability to work overnight 12.5-hour shifts
• Strong clinical assessment and documentation skills
• Physical ability to assist with patient care tasks (lifting, repositioning, standing)
• Compliance with onboarding and credentialing requirements
• Valid state or federal ID required
• COVID-19 vaccination or approved exemption required
• Prior experience in long-term care or skilled nursing preferred
• Local candidates only
