Job Overview We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a local contract assignment. The LPN will provide direct patient care under the supervision of Registered Nurses and physicians while supporting residents in a skilled nursing and long-term care environment.

Key Responsibilities
- Provide direct nursing care to residents in accordance with care plans
- Administer medications and treatments as prescribed
- Monitor and document patient conditions and report changes promptly
- Assist with admissions, discharges, and patient assessments
- Collaborate with physicians, Registered Nurses, and interdisciplinary team members
- Maintain accurate and timely clinical documentation
- Ensure compliance with facility policies and regulatory standards
Requirements
- Active Licensed Practical Nurse (LPN) license
- Current BLS certification (if required by facility)
- Strong clinical assessment and patient care skills
- Excellent communication and interpersonal abilities
- Skilled nursing, rehabilitation, or long-term care experience preferred
- Ability to work independently and within a multidisciplinary team
