REQUIREMENTS
Active Licensed Practical Nurse (LPN/LVN) License in good standing
Current BLS certification preferred
Previous experience in skilled nursing, rehabilitation, long-term care, or related healthcare settings preferred
Strong clinical assessment and medication administration skills
Excellent communication and documentation abilities
Ability to work effectively within an interdisciplinary healthcare team

KEY RESPONSIBILITIES
Provide direct nursing care in accordance with physician orders and individualized care plans
Administer medications, treatments, and nursing interventions safely and accurately
Monitor resident conditions and promptly communicate changes to the healthcare team
Perform nursing assessments and maintain accurate patient records
Assist with the development and implementation of individualized care plans
Collaborate with physicians, registered nurses, therapists, and support staff
Maintain accurate and timely clinical documentation
Ensure compliance with infection-control procedures, safety standards, and regulatory requirements
Educate residents and families regarding treatments and care plans
Promote resident comfort, dignity, safety, and overall quality of life

Competitive pay rate of $30.00 - $35.00 per hour
