Job Overview
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) for a local contract assignment. The ideal candidate will provide direct nursing care, administer medications and treatments, monitor resident conditions, and collaborate with the healthcare team to deliver exceptional patient-centered care.
Key Responsibilities
Provide direct nursing care in accordance with physician orders and individualized care plans.
Administer medications, treatments, and nursing procedures safely and accurately.
Monitor, assess, and document resident conditions and responses to care.
Communicate changes in resident status to physicians and healthcare team members.
Assist with admissions, transfers, and discharge planning.
Maintain accurate and timely clinical documentation.
Follow infection prevention, safety, and regulatory standards.
Support quality improvement initiatives and resident-centered care practices.
Promote resident comfort, dignity, and overall well-being.
Requirements
Active Licensed Practical Nurse (LPN/LVN) license required.
Current BLS Certification preferred.
Previous long-term care, rehabilitation, or skilled nursing experience preferred.
Strong medication administration and clinical assessment skills.
Excellent communication and organizational abilities.
Ability to work effectively in a fast-paced healthcare environment.
