Rates:
Malpractice Must be included in rates.
Orientation $150-157.50 - half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly $300-315 - Per hour, not OT
Holiday $450-472.50 - Must be 1.5x the hourly rate. Per holiday hour, no OT

Duties: Provide anesthesia
Reason for Coverage: Employed CRNA on maternity leave and one CRNA resigned.
