Rates for submission:
Malpractice Must be included in rates.
Orientation ***-110 - half the hourly/callback rate*** kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly ***-220 - Per hour, not OT
Holiday ***-330 - Must be 1.5x the hourly rate*** Per holiday hour, no OT
Schedule/Availability Requirements:
Assignment: T-F and needs to be able to provide consistent coverage for up to 12 months

Duties: Primary care provider covering ages newborn to geriatric care. - we are looking for a provider to service mainly 18 years and above but may be asked to occasionally see teenagers.
Required Procedures: primary care procedures; circumcisions potentially
EMR: eClinicalworks
Support Staff: 1 MA, receptionist; clinic RN as needed
Reason for Coverage: Have not been able to fill full time need
Deal Breakers? not being available T-F and needs to be able to provide consistent coverage
