Summary

The hiring salary for this position is $62,723.06 annually.

Telework eligibility is determined by county policy and the approval of the hiring department.

Charles County Government is seeking an experienced Long Term Care Coordinator to perform a wide variety of professional work including case management for clients enrolled in the Group Subsidy Housing program, and various other state and federal assistance programs. Coordinator will provide comprehensive management and coordination of services to those legally designated to the Chief of Aging and Human Services under the state Public Guardianship program.

Essential Job Functions

  • Maintains statistical database and confidential client case files including preparing reports to the Circuit Court and presenting to the Guardianship Review Board.
  • Represents the Chief of Aging and Human Services under the state Public Guardianship program in legal proceedings and court appearances.
  • Develops alternatives to guardianship and works with social services colleagues regarding potential wards and works through all the phases pre-guardianship in each case
  • Determines eligibility and enrolls new clients in the Group Housing Subsidy Program
  • Monitors program requirements, invoicing, budget management, and compliance for assigned programs.
  • Maintains client records, statistical data, and all state and federal program reporting
  • Serves as case manager for assigned clients enrolled in various state and federal assistance programs
  • Guardianship case management billing for assigned wards.
  • Performs other related job duties as assigned.

Qualifications, Knowledge, Skills, and Abilities

Education and Experience:

Bachelor's degree in human services, geriatrics, social work, behavioral science, health care, or related field. Three (3) years of related work experience; or an equivalent combination of education, experience and training.

Licenses or Certifications:

Must possess a valid driver's license

Licensed Social Worker or RN preferred

Work is subject to frequent interruptions

Knowledge, Skills and Abilities:

  • Knowledge of community resources and public benefit programs.
  • Knowledge of health conditions and sociology of the older adult population and family caregivers.
  • Knowledge of effective methods and techniques of counseling.
  • Knowledge of elderly population.
  • Knowledge of PC operations.
  • Ability to counsel older persons.
  • Ability to investigate complaints regarding care and other matters and take or recommend appropriate actions to resolve such matters.
  • Ability to interpret state and federal regulations.
  • Ability to communicate effectively orally and in writing.
  • Ability to establish and maintain effective working relationships with others encountered in the work.
  • Ability to initiate own daily assignments, follows through on matters, and uses independent judgment and training to take appropriate actions to deal with standard recurring situations.

PHYSICAL DEMANDS

The work is sedentary with frequent periods of walking and standing. Typical positions require workers to lift and carry up to 50 pounds; climb stairs; bend and crouch; reach, hold, grasp and turn objects; and use fingers to operate computer or typewriter keyboards; and the ability to speak normally, to use normal or aided vision and hearing.

WORK ENVIRONMENT

Principal duties of this job are performed in a general office environment. Field work in community facilities and client homes as needed. This position may come in contact with bodily fluids, proper safety supplies will be provided.
