Experience working with the elderly and those with dementia/Alzheimer's Disease, preferred work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.Licensure: State of Connecticut Nursing License; CPR certificationBilingual Spanish Prefered Job Standards1. Conduct new client intakes and assessments to determine the appropriateness of referrals and encourage participation at start of program and once enrolled.2. Conduct/assess the nutritional, medical, physical and mental functional needs of the clients during the initial assessment and ongoing as necessary within scope of nursing practice.
