Responsibilities:

• Provide direct care to residents, including administering medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with residents, families, and healthcare teams

• Conduct routine assessments and evaluations to ensure resident health and well-being

• Maintain accurate and up-to-date records and reports

• Participate in interdisciplinary team meetings and contribute to care plan development

• Demonstrate a commitment to resident safety and prevent adverse events

Requirements:

• Completion of a state-approved training program

• Current LPN license (pass NCLEX-PN exam)

• BLS certification
