General Summary: Responsible for the accurate and timely administration of medications, treatments, documentation, care coordination, supervision of nursing assistance and medication aids, and emergency response during assigned shifts.
Essential Job Duties:
- Directs the day-to-day functions of the nursing assistants and medication aides in accordance with rules, regulations, and guidelines that govern the long-term care facility.
- Ensure staffing sheet and assignment are completed for the shift.
- Participate in shift-to-shift reports with the team.
- Ensure controlled substances are counted and without discrepancies in accordance with procedures.
- Ensure nursing personnel comply with nursing procedures in the facility.
- Participates in facility surveys (inspections) made by authorized government agencies as requested.
- Admit, transfer,r and discharge residents as required.
- Conduct nursing assessment(s) with resident and initiate responsive action.
- Conduct resident medication reconciliation.
- Assist in planning the nursing services portion of the resident’s discharge plan.
- Complete accident and incident reports, as necessary.
- Complete and forward resident notifications to the appropriate departments as needed.
- Maintain daily census.
- Contact MD/NP/RP in response to resident needs or change of condition.
- Receive and process telephone and verbal orders form prescribers.
- Transcribe prescriber’s orders to resident order sheets, MAR, TAR, care plans, etc. as appropriate.
- Provide leadership to nursing personnel on your unit/shift.
- Make daily rounds ensuring nursing personnel are performing work assignments in accordance with standards of care. Report concerns to supervisor.
- Administers prescribed medications and treatments inclusive of but not limited to dressings, catheterization, suctioning, oxygen, etc.
- Process laboratory findings and reports in accordance with standards of care.
- Consult with prescribers in response to resident presentations, treatment, rehabilitation, etc.
- Provide direct resident care as needed.
- Obtain and process prescribed lab specimens as ordered.
- Coordinate prescribed diagnostics for the resident.
- Inform families and other contacts of the death of a resident.
- Ensures compliance with Infection Control standards.
- Maintain accurate and complete 24-hour reports.
- Ensure an adequate supply of PPE is readily available for personnel use.
- Ensure the unit/resident rooms are maintained in a clean, safe, and sanitary manner.
- Ensures nursing assistants and medication aides follow resident care plans and care cards, ensuring timely documentation.
- Interacts with residents, families, and visitors to solve problems maintaining goodwill which promotes positive customer relations.
- Maintains the self-respect, personal dignity, and physical safety of each patient
- Assures continuity of services and care
- Attends in-service training and education sessions, as assigned; completes Relias training on time every month.
- Must be familiar with emergency procedures and may be required to assist with evacuations.
Other Duties:
- May require flexible hours (weekends, evenings, and holidays).
- Must have reliable transportation.
- Able to lift and/or carry objects more than 25lbs.
- Utilize outstanding phone skills.
- Utilize excellent computer skills. Should be able to effectively use Microsoft Office Suite, MatrixCare, and all proprietary and user-end applications required for the successful completion of position duties
- Operate as the member of a team both within the department and across departments.
- Works beyond normal working hours on weekends and holidays and other shifts, as necessary.
- Subject to call-back during emergency conditions.
- Considered essential personnel during emergencies such as, but not limited to, pandemics.
- Perform other duties as assigned.
