About the Role:

The Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality patient care within the long-term care settings. This position involves providing direct nursing care under the supervision of registered nurses and physicians, ensuring patient comfort, safety, and well-being. The LPN will be responsible for monitoring patient health, administering medications, and assisting with daily living activities, contributing to comprehensive care plans. Minimum Qualifications:

• Current and valid Licensed Practical Nurse (LPN) license in the state of Minnesota.
• Completion of an accredited practical nursing program.
• Basic Life Support (BLS) certification.
• Demonstrated knowledge of nursing principles, patient care techniques, and medication administration.
• Strong communication and interpersonal skills to interact effectively with patients and healthcare team members.

Preferred Qualifications:

• Experience working in a clinical or long-term care setting within Minnesota.
• Familiarity with electronic health record (EHR) systems.

Responsibilities:

• Provide basic nursing care to patients, including monitoring vital signs and reporting changes to the healthcare team.
• Administer prescribed medications and treatments accurately and safely according to established protocols.
• Assist patients with activities of daily living such as bathing, dressing, and mobility to promote independence and comfort.
• Document patient information, care provided, and observations accurately in medical records to ensure continuity of care.
• Collaborate with registered nurses, physicians, and other healthcare professionals to implement and update individualized care plans.
• Educate patients and their families on health maintenance, disease prevention, and post-treatment care instructions.
• Maintain a clean, safe, and organized work environment in compliance with healthcare regulations and standards.
