The licensed practical nurse LPN assists the RN with assessing planning implementing and evaluating patient care. The LPN is responsible for documentation of their care including interventions outcomes and patientfamily education. They collaborate with physicians and other members of the interdisciplinary health care team to provide continuity of care. The LPN is responsible for the direction and assignment of nursing interventions that may be performed by others as well as care and interventions delegated to them by the RN. Must demonstrate the knowledge and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization. Demonstrates ability to appropriately modify approach and procedures to meet needs of agediversity of population served for the following age groups Neonate birth - 28 days Infant 29 days - less than 1 year PediatricChild 1 year - 12 years Adolescent 12 years - 18 years Adult 18 years - 65 years Geriatric over 65 years Incorporates cultural considerations in the provisions of care. Knowledge of growth and developmental stages. Considers life changeseffects on health beliefs and behaviors. Provides necessary safety measures. Provides information and involves familycaregiver in decision making. 2. Competent use of the nursing process to assist the RN in assessing priorities formulating implementing evaluating and adapting the interdisciplinary plan of care to ensure quality of care is delivered. Performs delegated portions of the admission assessment in accordance with specific unit standards. Updates and documents patients plan of care once it is established by the RN. Upon admission and as appropriate assists the RN with patientfamilyothers discharge planning and teaching needs. Delegates and assigns appropriate patient care to aides. Recognizes changes in patient status based on interpretation of assessments. Reports findings appropriately and timely. 3. Demonstrates the skills and judgment necessary to implement medical plan of care nursing interventions and procedures as necessary for the care of the patient in collaboration with RN. Demonstrates knowledge of hospital policies and procedures for administering verifying and recording medications. Demonstrates knowledge of commonly used meds action dose and side effects. Functions successfully in an emergency situation. Maintains professional composure and confidence during stressful situations. Identifies patient care problems and communicates them to RN Handoff reflects current plan and status of the patient. 4. Maintains timely and accurate documentation of care provided to ensure quality care and the integration of information for use by the healthcare team. 5. Performs all responsibilitiesduties required by the Behavioral Health Unit as defined in the scope of practice to assure that the unique nature of the patient is addressed. This includes but is not limited to the age of the patient served. Initiates appropriate nursing action in crisis or emergency situations. Is aware of and can demonstrate the use of effective communication and de-escalation techniques with patients. Demonstrates the ability to apply restraintsseclusion when directed by a RN and follows policy and procedure. Determines the relative priority of selected events based on potential or actual risk to patients and unit effectiveness. 6. Assumes all other duties and responsibilities as necessary. Demonstrates willingness to identify andor assume activities relative to the developmental needs of the unit and the nursing department Demonstrates dependabilityflexibility in meeting scheduling needs of the unit and hospital. Demonstrates willingness to float to other units as needed. Accepts assignment as a preceptor for new staff. Accepts responsibility for mentoring healthcare students. Hours will be dependent on patient census and workload.
