Responsibilities

• Provide licensed practical nursing care to patients following care plans.

• Accurately document patient assessments, treatments, and outcomes in EHR systems.

• Monitor patient vital signs and report changes to the supervising nurse promptly.

• Administer medications and treatments as prescribed, observing safety protocols.

• Collaborate with a multidisciplinary team to coordinate patient care plans.

• Meet performance targets through efficient patient throughput and quality care.

• Maintain compliance with regulatory standards and quality assurance protocols.

• Proactively identify improvement opportunities and contribute to process optimizations.

• Educate patients and families about care plans, procedures, and post-discharge needs.

• Demonstrate accountability by delivering own results within a team-driven environment.

• Participate in on-site training programs to support professional growth and role expansion.

Experience/Qualifications

• Current LPN license in the applicable state with active registration

• Accurate medication administration following prescribed protocols

• Proven ability to document assessments and interventions in an electronic health record

• Vital signs monitoring and prompt escalation to supervising nurse

• Compliance with regulatory standards and quality assurance protocols

• Collaborative, multidisciplinary teamwork to coordinate care plans

• Commitment to efficiency and achieving throughput targets

• Proactive identification of process improvements and participation in optimization

• Patient and family education on care plans, procedures, and post-discharge needs

• On-site training participation and ongoing professional development

• Strong accountability for individual results within a team-driven environment

• Experience with on-site onboarding and adherence to shift schedules

• Ability to thrive in a growth-oriented, merit-based environment
