Licensed Practical Nurse (LPN) – Weekend On-Call
Pleasant Hill Senior Services

Schedule:

  • On-call from Friday 5:00 PM through Monday 8:00 AM
  • Primarily phone-based with limited in-person visits as needed

Position Summary:
Pleasant Hill Senior Services is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide on-call coverage for our residents over the weekend. This unique role is primarily focused on telephone triage and medication approvals, with on-site visits for urgent needs such as falls, dressing changes, and changes in condition.

Key Responsibilities:

  • On-Call Coverage (Phone)

    • Respond promptly to calls from staff and/or families regarding resident needs from Friday 5:00 PM to Monday 8:00 AM.
    • Provide clinical guidance and approve routine and PRN (as needed) medications per standing orders and agency protocols.
    • Triage symptoms or concerns over the phone and determine appropriate next steps (home care, urgent visit, or emergency services).
  • In-Person Visits (Fri., Sat & Sun, and as needed)

    • Conduct on-site visits for urgent or emergent resident needs, including but not limited to:
      • Falls or suspected injury
      • Dressing changes and basic wound care
      • Sudden or significant changes in condition (breathing changes, mental status changes, uncontrolled pain).
      • Death
    • Perform focused assessments, interventions within LPN scope of practice, and notify the on-call RN or provider as necessary.
    • Document all assessments, interventions, and communications in accordance with agency policy and regulatory standards.
  • Communication & Coordination

    • Communicate changes in condition and weekend events to the interdisciplinary team and/or primary nurse to ensure continuity of care.
    • Collaborate with on-call providers, pharmacists, and families as needed.
    • Escalate concerns appropriately and follow Pleasant Hill Senior Services policies and procedures.

Qualifications:

  • Current and unencumbered Licensed Practical Nurse (LPN) license in Arkansas
  • Experience in senior careassisted livinghome health, or long-term care strongly preferred.
  • Strong clinical assessment and triage skills, especially over the phone.
  • Ability to work independently, prioritize, and make sound decisions within LPN scope of practice.
  • Reliable transportation and ability to travel to residents’ locations for urgent weekend visits.
  • Current CPR certification (or willingness to obtain).

Role Details:

  • On-call role covering Friday 5:00 PM – Monday 8:00 AM each weekend or rotating weekends.
  • Majority of work is telephone-based, with only essential in-person visits.
  • Compensation: hourly and on-call pay.
  • Reports to: Director
