Job Description
We are looking for an LPN to join our Family Medicine Team as a Care Coordinator!
What Youll Do:
- Support Multiple providers
In this role, you will have the opportunity to help support patients by collaborating with patients to:
- Set healthcare goals
- Complete advance care plans
- Track providers quality goals
What Youll Bring:
- Completion of post secondary program in Practical Nursing
- Nursing degree
- Current Ohio LPN license
- Knowledge of Microsoft Office
- Preferred:
- Experience in physician offices
- BLS/CPR certified preferred.
- Proficient with technology required for data tracking and reporting
