Connected Health Care is partnering with a leading healthcare organization to recruit an experienced Home Health LPN Case Manager for a full-time opportunity in Meridian, Idaho. This position is ideal for a compassionate and dependable Licensed Practical Nurse who enjoys providing quality patient care in the home setting while coordinating services and supporting patients and families throughout their plan of care.
The Home Health LPN Case Manager will provide skilled nursing services, monitor patient conditions, collaborate with the interdisciplinary healthcare team, and help ensure patients receive safe, individualized care in the comfort of their homes.
Job Description
Provide skilled nursing care to patients in their homes according to physician orders and individualized plans of care.
Serve as an LPN Case Manager for an assigned home health patient caseload.
Perform patient assessments and monitor changes in physical, functional, and psychosocial status.
Provide treatments, medication administration, wound care, and other nursing services within the LPN/LVN scope of practice.
Monitor patient progress and communicate significant changes in condition to the supervising RN, physician, or appropriate healthcare professional.
Assist with developing and maintaining individualized patient care plans.
Coordinate patient care with Registered Nurses, physicians, therapists, home health aides, social workers, and other members of the healthcare team.
Educate patients and caregivers regarding medications, treatments, disease management, safety, and appropriate home care practices.
Reinforce teaching provided by the RN and other members of the interdisciplinary care team.
Provide compassionate support to patients and family members while promoting independence and quality of life.
Complete accurate and timely clinical documentation in accordance with agency, state, and federal requirements.
Monitor medication compliance and identify potential medication-related concerns.
Perform wound care and other ordered treatments while maintaining appropriate infection-control practices.
Communicate effectively with patients, caregivers, physicians, and members of the interdisciplinary team.
Participate in care coordination activities, case conferences, and other clinical meetings as required.
Help identify changes in patient needs and communicate recommendations to the supervising RN and healthcare team.
Follow all applicable home health regulations, organizational policies, and nursing standards.
Maintain patient confidentiality and protect sensitive health information.
Manage a field-based schedule efficiently while traveling between patient homes throughout the Boise area.
Education and Certification Requirements
Current and unrestricted Licensed Practical Nurse LPN/LVN license in Idaho or eligibility for Idaho licensure.
Minimum 1 year of Home Health experience required.
Graduation from an accredited Practical Nursing or Vocational Nursing program.
Current BLS/CPR certification for healthcare providers.
Valid driver's license required.
Reliable transportation required for travel to patient homes.
Ability to meet applicable automobile insurance requirements.
Strong clinical, organizational, communication, and documentation skills.
Preferred Qualifications
Previous experience as a Home Health LPN Case Manager or Home Health LPN.
Experience providing skilled nursing care in patients' homes.
Familiarity with Medicare home health services and documentation requirements.
Experience with wound care, medication administration, patient education, and chronic disease management.
Strong understanding of care coordination and interdisciplinary healthcare.
Experience working with electronic medical records.
Ability to work independently while maintaining strong communication with the clinical team.
