Blue Ash Health & Rehab, LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join our team in Cincinnati, OH. This role involves providing compassionate and quality nursing care to our residents, ensuring their comfort and well-being in a supportive and collaborative environment.
Responsibilities
Assist in the delivery of nursing care to residents under the supervision of registered nurses and physicians.
Monitor and document patients’ health status, including vital signs and any changes in condition.
Administer prescribed medications and treatments accurately and safely.
Collaborate with healthcare team members to develop and implement individualized care plans.
Provide support and education to residents and their families regarding health and wellness.
Maintain accurate and timely medical records in compliance with company policies and legal regulations.
Ensure a clean, safe, and comfortable environment for residents.
Requirements
Current and valid LPN license in the state of Ohio.
Proven experience working as an LPN in a healthcare or rehabilitation setting preferred.
Strong knowledge of nursing practices, procedures, and standards.
Excellent communication and interpersonal skills.
Ability to work effectively both independently and as part of a multidisciplinary team.
