What You'll Do
Manage incoming referrals and coordinate admissions
Review referrals for clinical appropriateness
Communicate with patients, families, physicians, hospitals, and referral sources
Verify insurance benefits and authorizations
Coordinate seamless transitions of care
Maintain accurate documentation and patient records
Build and maintain strong relationships with referral partners
Qualifications
Current Washington State LPN license
Healthcare experience required; home health, hospice, intake, referrals, or care coordination experience preferred
Strong communication, organization, and customer service skills
Ability to multitask and work independently in a fast-paced environment
EMR and computer proficiency
