AuthoraCare Collective is seeking a LPN/CMA Care Coordinator House Calls. This position is Full-time: Monday - Friday 8:00am - 5:00pm supporting all of our service areas. Office base/High Point.
A care coordinator is a healthcare professional responsible for managing and coordinating all aspects of a patient's care, including assessing their needs, developing individualized care plans, communicating with health providers, gathering data from practice EMR systems as well as patient/caregiver calls, and ensuring they access necessary services and support to achieve optimal health outcomes; essentially acting as a central point of contact for patients to navigate their healthcare journey.
As a part of our incredible team of care coordinators, you will build strong relationships with patients through monthly calls, supporting them with services such as Chronic Care Management (CCM), Behavioral Health Integration (BHI), Principal Care Management (PCM), and Remote Patient Monitoring (RPM). You will empower patients to meet their goals, guide them to their healthcare journey, and advocate for their needs.
