Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Conduct thorough assessments, identify areas of risk, and develop interventions to prevent complications

• Document patient information accurately and thoroughly in accordance with facility policies

• Maintain open communication with patients, families, and colleagues to ensure continuity of care

• Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction

Requirements

• Current LPN licensure in the state of Ohio
