Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated LPN Coordinator/Dispatcher to work with us in our West Palm Beach location to provide coordination and dispatch of our staff visits on the following schedule: Monday - Friday 8:00am - 4:30pm.

Job Summary

The LPN Coordinator is responsible for coordinating with Managers and team members in patient/family direct and indirect care needs and provides quality professional intervention based on protocols and policy regarding the physical, emotional and psychological needs. Interacts with management, physicians and field staff utilizing a teamwork approach. Serves as a professional leader in assessing, planning, guiding and evaluating patient care activity. Pledge to make the comfort and care of patients and families the highest priority. Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times. Ensure the complete satisfaction of the patient and family.

Requirements

  • Current license to practice as a Licensed Practical Nurse in the State of Florida
  • Basic Life Support Certification
  • Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing
  • Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills and needs
  • Basic computer skills and EMR software experience (a plus)
