We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team. The position requires a compassionate individual who will provide exceptional care to residents in a day shift setting. This is a full-time position with a schedule of 40 hours per week.

Responsibilities

  • Administer medication and perform treatments on an assigned unit as ordered by the physician and within the scope of the license. This includes a variety of medication forms and treatments such as wound care, tracheostomy care, and catheter care.
  • Observe and report the effectiveness of medications and treatments.
  • Ensure an adequate amount of supplies and medication are available.
  • Receive and deliver information regarding residents' medical, social, and functional needs.
  • Participate in shift reports to ensure continuity of care.
  • Communicate with residents, families, and staff to ensure comprehensive care.
  • Complete initial admission nursing assessments for newly admitted residents.
  • Assist with the development of MDS assessments, ongoing quarterly reviews, and care plans.
  • Maintain accurate clinical and administrative documentation.
  • Participate in the Quality Assurance and Performance Improvement (QAPI) program.
  • Attend and participate in staff meetings.

Knowledge and Skills

  • Understanding of gerontological and geriatric nursing theory and practice.
  • Familiarity with the effects of the aging process.
  • Knowledge of team nursing practices and procedures.
  • Experience in implementing treatment plans for resident/patient care.
  • Proficiency in pharmacology and medication management.
  • Compliance with federal, state, and agency laws governing nursing practices.
  • Strong observation and reporting skills.
  • Effective oral and written communication abilities.
  • Interpersonal skills to establish and maintain effective work relationships.
