Job Summary:
Tryfacta is seeking a LPN Days 36 GTE for our client in Grand Junction, CO 81501. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: LPN Days
Location: Grand Junction, CO 81501
Duration: 13 weeks (Possible extension)
Work Schedule: 12s Variable – Days

Responsibilities for this position include, but are not limited to:

Provide direct nursing care to residents in a Skilled Nursing Facility (SNF)/Long-Term Care (LTC) setting.
Assess, monitor, and document residents' health status and report changes to the healthcare team.
Administer medications, treatments, and therapies as prescribed by the physician.
Perform wound care, medication administration, and other nursing procedures according to facility policies.
Collaborate with physicians, nurses, CNAs, and interdisciplinary team members to deliver quality resident care.
Maintain accurate and timely documentation using the facility's electronic medical record (EMR) system.
Ensure compliance with infection control, patient safety, and regulatory standards.
Respond appropriately to emergencies and changes in resident condition.
Support residents with individualized care plans while promoting comfort, dignity, and independence.
Provide compassionate, resident-centered care in a fast-paced Skilled Nursing environment.

To be considered for this position, you should have: [Skills, Education, or Experience]

Active LPN License in the state of practice (or compact license, if applicable).
Minimum 1 year of Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience required.
Must be a reliable, dependable, and strong candidate with excellent clinical skills.
Must be comfortable working 12-hour day shifts .
Weekend availability is required as needed (Thursday, Friday, and Saturday schedule).
Current BLS Certification preferred or required per facility policy..
