Job Summary:
Tryfacta is seeking a LPN Days 36 GTE for our client in Grand Junction, CO 81501. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: LPN Days 
Location: Grand Junction, CO 81501
Duration: 13 weeks (Possible extension)
Work Schedule: 12s Variable – Days

Responsibilities for this position include, but are not limited to:
  • Provide direct nursing care to residents in a Skilled Nursing Facility (SNF)/Long-Term Care (LTC) setting.
  • Assess, monitor, and document residents' health status and report changes to the healthcare team.
  • Administer medications, treatments, and therapies as prescribed by the physician.
  • Perform wound care, medication administration, and other nursing procedures according to facility policies.
  • Collaborate with physicians, nurses, CNAs, and interdisciplinary team members to deliver quality resident care.
  • Maintain accurate and timely documentation using the facility's electronic medical record (EMR) system.
  • Ensure compliance with infection control, patient safety, and regulatory standards.
  • Respond appropriately to emergencies and changes in resident condition.
  • Support residents with individualized care plans while promoting comfort, dignity, and independence.
  • Provide compassionate, resident-centered care in a fast-paced Skilled Nursing environment.

To be considered for this position, you should have: [Skills, Education, or Experience]
  • Active LPN License in the state of practice (or compact license, if applicable).
  • Minimum 1 year of Skilled Nursing Facility (SNF) or Long-Term Care (LTC) experience required.
  • Must be a reliable, dependable, and strong candidate with excellent clinical skills.
  • Must be comfortable working 12-hour day shifts .
  • Weekend availability is required as needed (Thursday, Friday, and Saturday schedule).
  • Current BLS Certification preferred or required per facility policy..
