Licensed Practical Nurse (LPN)
**Job Summary:**
The Licensed Practical Nurse (LPN) provides compassionate, patient-centered nursing care under the direction of a Registered Nurse or physician. The LPN is responsible for administering medications, monitoring patient conditions, performing nursing procedures, documenting care, and collaborating with the interdisciplinary healthcare team to promote positive patient outcomes in accordance with clinical standards and regulatory requirements.
**Requirements:**
* Graduate of an accredited Practical Nursing program.
* Current Licensed Practical Nurse (LPN) license in the state of practice.
* Current BLS certification preferred or required per facility policy.
* Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.
**Responsibilities:**
* Provide direct nursing care and implement individualized care plans.
* Administer medications and treatments as prescribed.
* Monitor, assess, and document patient conditions, reporting changes to the Registered Nurse or provider.
* Assist with admissions, transfers, discharges, and ongoing patient care.
* Maintain accurate and timely clinical documentation.
* Collaborate with physicians, Registered Nurses, and the interdisciplinary healthcare team.
* Follow infection prevention, patient safety, HIPAA, and regulatory standards.
* Educate patients and families within the scope of practice.
* Perform other duties as assigned.
