Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)

Summary

We are seeking a compassionate and dedicated LPN/LVN to provide quality patient care in a long-term care and geriatric setting. The ideal candidate will administer medications and treatments, monitor resident conditions, assist with care planning, and collaborate with the interdisciplinary team to ensure excellent patient outcomes.

Responsibilities

• Administer medications and treatments as ordered by the physician and within scope of license
• Perform treatments including wound care, tracheostomy care, enteral and parenteral medication administration
• Observe and report the effectiveness of medications and treatments
• Maintain adequate supplies and medication inventory
• Communicate effectively with residents, families, and healthcare team members
• Complete admission nursing assessments and assist with care plan development
• Participate in shift reports and maintain accurate clinical documentation
• Support Quality Assurance and Performance Improvement (QAPI) initiatives
• Respond appropriately to medical emergencies and patient care needs
• Follow facility policies, nursing standards, and safety procedures

Requirements

• Knowledge of gerontological and geriatric nursing practices
• Understanding of pharmacology and legal aspects of nursing care
• Strong observation, assessment, and documentation skills
• Excellent communication and interpersonal skills
• Ability to make independent clinical decisions
• Ability to maintain professional working relationships

Education

• Graduate of an accredited Practical Nursing Program

Certifications & Licensure

• Active LPN/LVN license required
• BLS certification preferred

Schedule

• 12-hour Night Shifts
• 6:00 PM – 6:30 AM
