About the Role: Better And Beyond Family Care is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Adult Day Care team in Cleveland, OH. Responsibilities:
Monitor and assess the health status of adult day care participants on a daily basis
Administer medications and treatments as prescribed by physicians
Maintain accurate and up-to-date patient health records and documentation
Coordinate with care team members to develop and implement individualized care plans
Respond promptly to medical concerns, emergencies, or changes in participant condition
Provide health education and guidance to participants and their families
Ensure a safe, clean, and supportive environment for all program participants
Requirements:
Valid Ohio LPN license in good standing
Prior experience in adult care, geriatric care, or a day program setting preferred
Strong knowledge of medication administration and clinical documentation
CPR and First Aid certification (or willingness to obtain)
Excellent communication and interpersonal skills
Compassionate, patient, and dependable with a team-oriented attitude
Ability to multitask and remain calm in fast-paced or urgent situations
