LPN - Group Homes

The Mary T Family of Companies is seeking a full-time Licensed Practical Nurse (LPN) in our ICF licensed group homes located in Coon Rapids, MN and surrounding suburbs.

The LPN is responsible for conducting a focused assessment of health status of an individual, and reporting changes and responses to interventions. Responsibilities also include implementing interventions and assigning and monitoring nursing tasks or activities to unlicensed direct support professionals (DSPs). A large part of this position will involve medical coordination, training unlicensed DSP staff, and the corresponding paperwork with these tasks. The LPN will also provide some direct nursing cares, however that will be a smaller function of the position.

Essential Job Functions:

• Through collection and comparison of data to normal findings and the individuals current health status, conduct a focused assessment of the health status of an individual. Required reporting of any changes and/or responses to interventions, in an ongoing manner, to a registered nurse or the appropriate licensed health provider for delegated or assigned tasks or activities.
• Participation with other health care providers, in the development and modification of a care plan
• Determining and implementing appropriate interventions, within a nursing plan of care, when delegated, ordered, or assigned by a registered nurse/licensed health care provider
• Assigning and monitoring nursing tasks/activities to unlicensed supportive personnel; promoting a safe and therapeutic environment; advocating for the best interests of the individual
• Assisting in the evaluation of responses to interventions
• Collaborating and communicating with other health care providers
• Providing health care information to individuals, staff or other interdisciplinary team members
• Providing input into the development of policies and procedures
• Accountability for the quality of care delivered, recognizing the limits of knowledge and experience; addressing situations beyond the nurses competency; and performing to the level of education, knowledge, and skill ordinarily expected of an individual who has completed an approved practical nursing program
• Responsible for the compilation and presentation of health care reports

Schedule:

• Full-time, daytime hours 9 AM - 5 PM
• Evening and weekend on-call triage responsibilities, in a rotation with the nursing team (approximately once every 5 weeks)

Position Requirements:

• Must be an active LPN licensed in Minnesota
• Experience in an ICF licensed group home is preferred
• Must possess a valid drivers license with a good driving record
• Must be able to pass a background check
• Strong verbal and written communication skills
• Basic computer skills
