Licensed Practical Nurse (LPN) Home Health

Location: Philadelphia, PA (Coverage Area: ZIP Codes 19104 & 19139)
Job Type: Contract
Job Summary

Penn Medicine Home Health is seeking a dedicated and experienced Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes throughout Philadelphia, PA, primarily covering ZIP codes 19104 and 19139. The ideal candidate will have a strong background in home health nursing and be highly proficient in wound care. This role requires excellent clinical skills, compassionate patient care, and the ability to work independently in a home health environment.

Responsibilities

  • Provide skilled nursing care to patients in their homes under the direction of an RN and physician.
  • Perform comprehensive patient assessments and monitor changes in patient condition.
  • Deliver and manage wound care treatments, including dressing changes and wound assessments.
  • Administer medications and treatments as prescribed.
  • Educate patients and caregivers on disease management, wound care, medications, and home safety.
  • Accurately document patient care and interventions in the EMR.
  • Collaborate with RNs, physicians, therapists, and other interdisciplinary team members.
  • Maintain compliance with Penn Medicine policies, CMS regulations, and home health standards.
  • Travel throughout the assigned coverage territory to complete scheduled patient visits.

Required Qualifications

  • Active Pennsylvania Licensed Practical Nurse (LPN) license.
  • Minimum 2 years of recent Home Health (Home Care) experience.
  • Strong wound care experience is required.
  • Current BLS Certification (AHA preferred).
  • Valid driver's license, reliable transportation, and auto insurance.
  • Excellent communication, assessment, and documentation skills.
  • Ability to work independently in the field.

Preferred Qualifications

  • Experience using home health EMR systems (EPIC experience is a plus).
  • Strong time management and organizational skills.
  • Experience managing a daily home health patient caseload.
