Job Description

We are seeking an experienced Licensed Practical Nurse (LPN) to join our Home Care team. The ideal candidate will have at least 2 years of home care experience and strong expertise in wound care. This role involves providing skilled nursing care to patients in their homes while promoting recovery, independence, and overall well-being.

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Perform patient assessments and monitor changes in condition.
• Deliver advanced wound care, including wound assessment, dressing changes, and infection prevention.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on wound management, medications, and disease management.
• Maintain accurate and timely clinical documentation.
• Collaborate with physicians, case managers, therapists, and other healthcare professionals to ensure quality patient care.
• Follow agency policies, state regulations, and infection control standards.

Requirements

• Active Pennsylvania LPN License.
• Minimum 2 years of Home Care experience.
• Strong wound care experience is required.
• Excellent assessment, communication, and documentation skills.
• Valid driver's license and reliable transportation (if required by the employer).
• Current BLS/CPR certification preferred.
