The scope of nursing includes direct patient care, and patient advocacy in the coordination of care directly with the RN case manager and with the multidisciplinary team.<\/span>

The duties include:<\/span><\/span><\/b><\/i>
<\/span><\/span><\/p>- Under the direction of an RN, provides patient care and identifies the physical, psycho -social, and environmental needs of patients<\/span>
<\/span><\/span><\/li> - Verifies Plan of Care prior to each visit and provides care according to the physician’s orders, assessment data, established standards and guidelines, and documents accordingly<\/span>
<\/span><\/span><\/li> - Incorporates short and long -term goals as established by the Plan of Care into nursing care, as evidenced by documentation in the nursing notes<\/span>
<\/span><\/span><\/li> - Performs an ongoing assessment during each visit and documents data<\/span>
<\/span><\/span><\/li> - Assesses patient/caregiver’s knowledge of disease process, treatment regimen, and/or medication administration<\/span>
<\/span><\/span><\/li> - Provides basic and specialized teaching as ordered in the Plan of Care and based on the patient’s needs<\/span>
<\/span><\/span><\/li> - Identifies and recommends other disciplines needed for quality care of the patient<\/span><\/span>

Requirements<\/h3>- Active LPN license in Virginia.<\/span><\/span>
<\/span><\/li> - Minimum of one year of acute nursing experience or skilled home health experience.<\/span><\/span>
<\/span><\/li> - Must have a driver’s license and current CPR certification.<\/span><\/span>
<\/span><\/li> - Ability to read/write and communicate in English.<\/span><\/span>
