Under the guidance of a Registered Nurse and physician orders, you’ll deliver skilled nursing services that support healing, dignity, and independence.
Position Summary
As a Home Health LPN, you will administer skilled nursing care on an intermittent basis in patients’ residences, following individualized plans of care and agency policies. You’ll play a vital role in patient education, monitoring, and coordination of care across the healthcare team.
What You’ll Do
- Deliver skilled nursing care per physician orders and care plans
- Implement and evaluate nursing interventions in collaboration with RN
- Monitor patient conditions and report changes promptly
- Administer treatments, medications, and preventive/rehabilitative procedures
- Educate and support patients and families on care needs and self-care
- Maintain accurate, timely clinical documentation
- Participate in care conferences, audits, staff meetings, and in-services
- Assist physicians and RNs with specialized procedures
- Prepare equipment and maintain aseptic technique
- Provide on-call coverage (nights, weekends, and holidays as assigned)
Qualifications
- Graduate of a state-approved Practical/Vocational Nursing program
- Active LPN license in the state of Colorado
- IV Certification upon hire or within 6 months
- Minimum 1 year nursing experience (preferred)
- BLS Certification required upon hire

- Valid driver’s license and reliable personal vehicle
- Current auto insurance and proof maintained on file

- Operational phone or equivalent means of contact
